An In Case of Emergency (ICE) card is a convenient document containing essential personal and medical details. It aids emergency responders and medical professionals during accidents or medical emergencies. The card typically includes your name, emergency contacts, allergies, medical history, and treatment preferences. It’s advisable to carry one in your wallet, purse, or phone for easy access in emergencies.
PDM strongly recommends bringing your In Case of Emergency (ICE) card to our group walks.
If you don’t have one, please prepare your own In Case of Emergency (ICE) card or use our ICE card template, and bring it with you when attending our activities. (please keep it with you at all times!)

If you’re unable to download and print it, don’t worry. Please create your own ICE card by including the following information:
- Name
- Date of Birth
- Phone number
- Address
- Medical condition
- Allergies
- Medications
- Emergency Contact Details (Name, Relationship, Phone Number)
